Father came into our care for a period of respite, so you had an opportunity to sample our services. Unfortunately, at this assessment stage, your father's previous history of falling was not disclosed. After the period of respite had ended, the family asked for the placement to be made more permanent and to my knowledge, there were no concerns raised at this time. Sadly, your father’s mental health deteriorated very quickly, due to his advanced cognitive impairment, however, your father remained physically very active and quite fit. This was a huge challenge to the care team, as your father had no insight into the risks he presented, as we have a spacious environment. The care team sought advice from a range of professionals (the Community Mental Health Services, Physiotherapy services, the Falls team, Adult Social Care Safeguarding Team, the Care Quality Commission) and it was agreed that to keep your father safe that he would need to be moved to a safer nursing environment.
