Job context

This role is a team leader role, with clear responsibility for leading the care team in the dleivery of care and assisting the nursing team, undertaking assessments. observations and medication administration as required. Reviewing and writing of care plans ensuring they are implemented, participating in best interests and family meetings..

Key measures

1. Mandatory training 
2. Supervision meetings with Line Manager every 3 months.
3. Results and feedback from home inspections and audit reports (internal and external) participating in actions plans as appropriate
