Inquest into death of woman after care home fall

Care home provider Four Seasons Health Care, have disciplined three of their care workers in relation to a death that occurred at The Elms in Louth. Though the death is officially recorded as an accident there were a series of safety failings from staff that suggest the fall might have been avoided.
