Are you a senior nurse with proven leadership experience and a passion for delivering exceptional care in complex dementia and discharge to assess pathways? We are looking for a Clinical Lead to join our specialist team at Schoen Clinic York.

As Clinical Lead, you will oversee and coordinate the care delivery for residents with complex dementia on our Derwent ward and within the Discharge to Assess (D2A) pathway on our Ryburn ward. You’ll manage the full Multidisciplinary Nursing Team, and work with the senior nurses to inspire and lead them to manage the senior care assistants and care assistants. You will drive forward training and development for your team whilst ensuring best practice is followed, risk is managed effectively, and patient safety remains paramount.

Key Responsibilities: (although not limited to)

·  Lead and support the nursing teams across the dementia and D2A services.

·  Ensure consistent delivery of safe, high-quality, patient-centred care.

·  Oversee clinical governance, audits, and continuous improvement initiatives.

·  Oversee and review care plans, risk assessments and medication management to ensure safe and effective treatment.

·  Collaborate with the General Manager, Deputy Manager, other MDT team members, and external teams.

·  Act as the senior clinical point of contact in the absence of the General Manager.

·  Mentor and develop staff, promoting a positive, supportive working environment.

About You:

As the Clinical Lead, you will be a Registered Nurse (RGN, RNLD or RMN) with active NMC registration and no restrictions on practice and have proven clinical leadership experience in dementia care and/or discharge planning. You will have a strong understanding of safeguarding, risk management, and CQC standards and work with the relevant teams to ensure this is upheld at all times.

You will be a confident, compassionate communicator and natural team motivator.
