Job Summary
We are seeking an experienced, dedicated and compassionate
Senior Health Care Assistant to join our care team in the capacity of
a Team Leader. The ideal candidate will be responsible for providing
high-quality care for our residents, ensuring the well-being of
residents, and collaborating with their friends, relatives and other
healthcare professionals to deliver excellent care outcomes. As a Team
Leader, you will play a vital role in assessing residents' needs,
developing care plans, carry out regular care plan reviews.
Duties
- Conduct comprehensive assessments of residents' care needs, develop and implement personalised care plans based on their assessed needs and preferences.
- Administer medications and support the nursing team in the provision of care to meet the healthcare needs of our residents.
- Provide support and provide relevant information to residents and their families regarding their care.
- Collaborate with multidisciplinary teams to ensure coordinated care delivery.
- Maintain accurate and up-to-date resident records in compliance with regulatory standards.
- Uphold infection control protocols and maintain a safe environment for the elderly
- Carry out audits in accordance to organisation quality assurance system.
- Carry out regular supervision for the care team.
Skills / Qualifications:
- NVQ Level 3 Health and Social Care
- Strong knowledge of older people’ care principles and practices
- Excellent communication skills
- both verbal and written
- with the ability to build relationship with older people including people living with dementia
- Demonstrated ability to work effectively in a fast-paced environment while maintaining attention to detail
- Compassionate nature with a strong commitment to providing high-quality care
- Previous experience in providing nursing care for older people including those living with dementia within a Nursing Home setting is essential for this role
