Admission Criteria

  • Admissions Policy – The Hamptons Care Home 1. Pre?Admission & Referral Handling • • Accept referrals from authorities, hospitals, families, and healthcare professionals. • • Review referral details promptly—aim to decide on acceptance within 1 hour of receipt. • • Schedule a pre-admission visit or tour, and provide families with a welcome pack including a Statement of Purpose and a resident handbook. 2. Assessment & Acceptance • • Admission is based on a needs assessment and suitability for existing residents. • • Emergency or unplanned admissions are accepted swiftly, with documentation completed within 24 hours. • • The resident or their representative must sign consent and placement agreements. 3. On the Day of Admission • • A dedicated staff member (Senior Carer or Manager) greets the new resident and family. • • Complete all required paperwork, including consents, medical info, dietary needs, and belongings inventory. • • Provide a room tour, explain facilities and emergency procedures. 4. Induction & Orientation • • Provide a welcome booklet and key information pack. • • Introduce the resident to staff, fellow residents, and key areas. • • Assign a key worker for personalised care planning. • • Review care plans within 1 day (or within 2 hours if urgent). 5. Medication & Medical Setup • • Hospital discharges must provide 14 days’ worth of medication. • • Record all prescribed medications and ensure medical history is documented. • • Liaise with the GP or local health team for continuity of care. 6. Privacy, Dignity & Safety • • Support personal care in a respectful, individual-centred manner. • • Maintain resident confidentiality and promote independence. • • Enforce policies regarding smoking, alcohol, safeguarding, and possession limits. 7. Documentation & Record Keeping • • Maintain an Admission Register with all relevant referral and admission details. • • File all essential documentation, including care plans, assessments, and inventories. • • Ensure prompt review and escalation for emergency admissions. 8. Discharge Protocols • • Use clear criteria for discharge, documented in care plans. • • Return any personal allowance funds held on the resident’s behalf. • • Coordinate with social workers or care teams to ensure safe transfer.
