The care home

Heartly Green is a lovely, 59 bedded purpose-built residential care home for older people and is situated in a quiet residential area of Irlam in Salford.

Heartly Green is a warm, welcoming and friendly home with some wonderful places to relax in and enjoy company of family and friend, including lounge areas and some themed areas for residents to enjoy quiet moments as well a large accessible garden.

All our bedrooms are comfortable and spacious with en-suite facilities; we encourage residents to personalise their rooms to make them more homely.

Key Responsibilities:

·  ·  Plan, develop, and coordinate a diverse range of social, recreational, educational, and leisure activities that meet the interests, abilities, and support needs of participants.

·  ·  Engage service users in meaningful activities that promote social interaction, wellbeing, independence, and quality of life.

·  ·  Assess participant needs, preferences, and abilities to ensure activities are inclusive, accessible, and person-centred.

·  ·  Monitor participant engagement and behaviour during activities, documenting observations, outcomes, and progress in accordance with organisational and clinical reporting requirements.

·  ·  Conduct basic risk assessments and ensure all activities comply with health, safety, and organisational policies and procedures.

·  ·  Provide direct support to individuals with physical, cognitive, or psychosocial impairments to enable safe and comfortable participation.

·  ·  Collaborate with support staff, healthcare professionals, families, and external providers to coordinate activities and support participant goals.

·  ·  Adapt activities as required to accommodate changing needs, abilities, or environmental factors.

·  ·  Manage activity schedules, resources, and materials, ensuring efficient use of time and budget.

·  ·  Gather and respond to participant feedback to continuously improve activity programs.

·  ·  Promote a positive, inclusive, and respectful environment that encourages participation and community engagement.
