We are excited to introduce a newly created Dementia Care Manager role, offering a unique opportunity to shape, lead and champion outstanding dementia care within our Shire Hall care home.
This pivotal role has responsibility for the leadership and management of our dementia community, ensuring the highest standards of relationship-focused care, meaningful occupation, and clinical excellence for people living with dementia. You will also act as the home’s dementia champion, leading learning, coaching and best practice across the wider team.

The Role: Dementia Care Manager

About the Role
As Dementia Care Manager, you will provide leadership across the dementia community, championing high-quality, relationship-centred care in line with regulations, evidence-based practice and Hallmark standards.
You will:

  • Lead the day-to-day operation of the dementia community, ensuring residents receive personalised, responsive care.
  • Act as the home’s lead for the Hallmark Dementia Strategy, embedding it into everyday practice and culture.
  • Oversee care planning, risk assessments and day-to-day dementia care delivery in partnership with nurses and care teams.
  • Play a key role in pre-admission assessments, family meetings and transition planning.
  • Work closely with the General Manager, Lifestyles Lead, Regional Dementia Practitioner and MDT to support residents’ health, wellbeing and engagement.
  • Lead and deliver dementia-specific learning, coaching and development across the home.
  • Build a strong community of practice, sharing learning both locally and regionally.

Quality, Governance & Compliance
You will drive continuous improvement by:

 

  • Ensuring the dementia community meets and exceeds CQC and Hallmark standards, with a clear ambition for ‘Outstanding’.
  • Supporting audits, action planning and quality improvement activity.
  • Maintaining accurate documentation, oversight of KPIs, and promoting Health & Safety and risk management.

Leadership & People Management
Working in partnership with the General Manager, you will:

 

  • Provide day-to-day leadership to nurses and care teams within the dementia community.
  • Ensure appropriate skill mix, staffing levels and capability to meet residents’ changing needs.
  • Oversee inductions, mandatory training and ongoing dementia-specific development.
  • Manage performance, appraisals and capability in a supportive and values-led way.

Family & Community Engagement
You will:

 

  • Build trusted relationships with families, GPs and external professionals.
  • Act as a source of dementia advice and reassurance.
  • Facilitate monthly family support meetings and develop dementia resources within the home.

Who We’re Looking For
Essential Criteria:

 

  • NVQ Level 4/5 (or equivalent) in Health & Social Care, or willingness to work towards
  • Minimum 4 years’ experience in long-term care of older people, including dementia care, ideally in a care home environment
  • Strong knowledge of evidence?based dementia care, care planning and end?of?life care
  • Previous leadership or supervisory experience
  • Experience in governance, quality frameworks and regulatory compliance
  • Knowledge of safeguarding, Health & Safety and inspection processes
  • Experience delivering training and coaching

Skills & Attributes:

  • Confident, compassionate leader with strong communication skills
  • Able to manage multiple priorities in a fast-paced environment
  • Good IT skills (Outlook, Windows, Excel)
  • Warm, resilient and empathetic
  • Passionate about meaningful activity, strengths?based dementia care, and open to learning the Montessori approach
