Care Coordinator – Domiciliary Team

Company: Roche Home Care

Location: LS11 office & community care areas (Birstall, Gomersal, Heckmondwike, Cleckheaton, Hightown, Scholes, Liversedge, Dewsbury, Shawcross)


Hours: Full-time - 8.30am – 5.00am – should the rota require it may be necessary for you to do some care calls (before or after these hours will be paid separately at Senior Carer rates)


Salary: £29,213per annum + on call (£30 weekdays & £60 weekends)


Car Owner/Driver Essential with clean driving licence – Mileage Paid

The Role

As a Care Coordinator, you will:

  • Work as part of our fantastic Domiciliary Team.
  • Collaborate with health professionals such as nurses and occupational therapists.
  • Provide management support to people who use our services.
  • Deliver person-centred care that empowers individuals to remain independent.

Requirements

  • Diploma Level 3/5 in Health & Social Care (or willingness to work towards).
  • Honesty, integrity, and strong observational skills.
  • Ability to remain calm in a crisis.
  • Punctuality and good attendance record.
  • Satisfactory DBS disclosure and safeguarding checks.
  • Flexibility to work evenings, weekends, and on-call rota.
  • Full UK driving licence and access to a car.
