Come and Join us for this presentation, Avoiding Falls and Preventing Hospitalisation, which has been designed to raise awareness of the risks older adults face and share practical ways to stay safe, healthy, and independent at home.
Led by Home Instead, they will share and explore the common causes of hospitalisation - from falls and missed appointments to poor nutrition and social isolation - and outline five key strategies to combat them.
The content also highlights how Home Instead Care Professionals support clients through companionship, nutrition, hydration, activity, and monitoring for early warning signs and how you can support your loved ones too. We share real-world examples, such as our partnership with South West Ambulance Service and the introduction of the Raizer Chair, which has helped dramatically reduce hospitalisation after falls.
By working together as families, communities, and care providers, we can reduce avoidable hospital admissions, ease pressure on the NHS, and most importantly, help older people live well, safely, and with dignity.
There will be delicious cream teas available too, all free of charge.
