Hazeldene – “Building trust and moving forward,” Hazeldene offers support to adults with an acquired brain injury in a friendly, empathetic residential service in Swansea. Staff provide responsive rehabilitation and support through a person-centred approach.
Hazeldene supports individuals to increase their skills through graded goal setting appropriate to individuals abilities in areas such as food shopping, personal care, using public transport, budgeting, social skills and self-medicating.
Tracscare employs qualified clinical support nurses and has behaviour advisors to support individuals at Hazeldene and their care team to work with improving communication, emotional and behavioural strategies.
