Meadowcroft provides nurse-led care for adults living with complex mental health needs, in a community-based care home.

The home has 18 en-suite bedrooms across two small units, each with a shared kitchen, lounge, dining area, and quiet space that promote familiar surroundings and meaningful connections.

There are also communal spaces including an activities hub, sensory room, and an accessible garden.

In the home, Connell Unit supports adults with functional mental health conditions who may display frequent or unpredictable behaviours of concern.

Some people may have a forensic background, a history of substance use disorder, or require Section 117 aftercare following hospital discharge. The structured environment offers high levels of behavioural support and observation, helping people to manage risks and build stability over time.

The home's other unit, Acorn, supports adults with similar diagnoses who don’t require intensive interventions or constant observation.

On Acorn, people benefit from a calmer environment with consistent routines and opportunities to socialise, often stepping down from more acute services, including Connell Unit.

Care is provided by a nurse-led team of general and mental health nurses, care professionals, and support staff trained in positive behaviour support.

The team works alongside each person to co-produce their care and recovery plans, encouraging independence, social connection, and meaningful activity.

The staff were very friendly, making me very welcome and were happy to answer any questions I had. They spoke in depth about future projects / activities involving the residents, which I think is important for stimulation and well-being for people at Meadowcroft. I think the staff have a wonderful infectious
enthusiasm for their work and are a great team!
I left the meeting happy and positive!

Exceptional care and wonderful staff. Recent illness for my relative meant that the staff and management were there for them 24 hours and we cannot thank them enough. Positive, calm and caring. Amazing!

When my friend first came in he was in a bad way. Your staff have done a fantastic job at getting him mobile, a little more self-sufficient and so much happier. This doesn't happen unless you have a fantastic team and it shows. I was worried about him initially but the staff here has given me no doubts
whatsoever that he is in the right place. During my first visits, my friend couldn't stand on his own, couldn't hold a conversation and was a shell of his former self, now I can see a glimmer of the person he used to be return.

Things need getting on top of from time to time, but the manager and staff are all willing to help to put things right. Most of the time I can sleep contented knowing my brother is getting well looked after.

Hi, I’m Carolyn, and I’m the Home Manager at Meadowcroft in Sutton-in-Ashfield.


Exemplar Health Care has a reputation for delivering first-class, person-centred care, which is achieved by truly focusing on those using the service. I wanted to get back to focusing on the individual and making a positive difference in their lives. This is something that Exemplar Health Care is known for.


I am thrilled to be able to develop what will no doubt be a strong and successful team at Meadowcroft.
