Pathways is a registered learning disability service that supports adults living with learning disabilities, autism, and associated mental and physical health needs.

It provides person-centred support that promotes dignity, independence, and choice, enabling people with complex needs to live safely and meaningfully in the least restrictive environment possible.

The service has eight large en-suite bedrooms, creating a homely and supportive setting that’s responsive to each person’s needs. People are encouraged to decorate and personalise their rooms to make the space their own.

There’s also an accessible sensory garden, activities hub, and communal kitchen and dining area for shared meals and social interaction.

People living at Pathways may have complex health needs such as diabetes, dysphagia, or epilepsy, and some require support with clinical interventions including catheters, Hickman lines, or PEGs.

The team also supports people with limited mobility, providing regular monitoring and repositioning to maintain comfort and wellbeing.

Many people move to Pathways from family homes or hospital settings when previous arrangements are no longer sustainable.

The structured environment provides stability and consistent support, helping people to build confidence before moving on to more independent settings such as supported living.

I was very impressed with the environment my friend lives in. It feels so homely when you walk in. Staff were very friendly and helpful especially the new manager. Staff always try to encourage people to take part in activities and there is always plenty of choices.

Always able to discuss issues with the staff. They take on board any concerns. Care is very good and support is excellent. Medical issues are always dealt with urgently and efficiently by senior staff and nurses.

Hi, I'm Andrew, the Home Manager at Pathways care home in Sutton-in-Ashfield.


Exemplar Health Care has a reputation for delivering first-class, person-centred care, which is achieved by truly focusing on those using the service. I wanted to get back to focusing on the individual and making a positive difference in their lives, this is something that Exemplar Health Care is known for.


I am thrilled to be able to develop what will no doubt be a strong and successful team at Pathways. However, I am most excited about sharing successes (big and small) with everyone who is a part of Pathways - we have an exciting journey ahead of us!
