Hylton Grange supports adults living with complex mental health needs, dementia, neuro-disabilities and physical disabilities. It’s part of Exemplar Health Care, one of the country’s leading nursing care providers.

The home provides complex nursing care from a team of trained Registered Nurses and Health Care Assistants. It has also has an in-house Physiotherapist and Occupational Therapist. Residents also have the support of Exemplar Health Care clinical experts, including Behaviour Support Practitioners, a Speech and Language Therapist and a Consultant Psychiatrist in times of crisis.

Hylton Grange isn’t your typical residential care home or supported living service. It supports people who are considered to have needs that are ‘too complex’ for mainstream care providers. For example, residents often have a number of secondary diagnoses and complexities including behaviours of concern, tracheostomies, ventilators and PEG feeding tubes.

Hylton Grange has 30 large bedrooms, each with an en-suite, across three ten-bed communities. This provides a homely and supportive environment that’s more responsive to people’s needs. Residents can personalise their bedroom if they wish. The home also has communal dining and living spaces, sensory bathrooms, as well as an activities hub, therapy room and large accessible gardens.

At Hylton Grange, daily life is all about choice. Activities are personalised to people’s interests, hobbies and goals. They focus on encouraging people to continue to do the things they enjoy, and enabling them to learn, volunteer and find new hobbies too if they wish.

The home has great community connections so people can remain part of their local area. You can often find residents visiting local attractions such as the Penshaw Monument, Herrington Park, the Winter Gardens or enjoying a show at the Empire theatre.

What can I say about Hylton Grange other than excellent. The staff make a care home and the staff here, from the management team to the cleaning staff treat the residents with respect, dignity and a caring nature. They are all friendly and supportive to relatives. Aesthetically it's the nicest home
I've been in. Lovely surroundings, big rooms, nicely decorated. I'm very choosy when it comes to my husband's care but I never regretted choosing Hylton Grange. Thank you to the manager and the team.

The staff are genuinely friendly and make me feel at ease.
I don't have any concerns about leaving my relative in their care.

Since my daughter has been staying at Hylton Grange she has been given lots of support and encouragement. The staff are all very friendly and approachable which I find very helpful.

Hi, I’m Donna and I’m the Home Manager at Hylton Grange in Sunderland.


Exemplar Health Care has a reputation for delivering first-class, person-centred care, which is achieved by truly focusing on those using the service. I wanted to get back to focusing on the individual and making a positive difference in their lives, this is something that Exemplar Health Care is known for.


I am thrilled to be able to develop what will no doubt be a strong and successful team at Hylton Grange. However, I am most excited about sharing successes (big and small) with everyone who is a part of Hylton Grange - we have an exciting journey ahead of us!
