Hylton Grange provides nursing care for adults living with complex needs arising from mental health and neurological conditions.

Located on Poole Road in Sunderland, the home offers 30 en-suite bedrooms across three small, purpose-designed units.

Each unit supports small-group living and includes shared kitchens, lounges, sensory bathrooms, and quiet spaces.

In addition to unit-level facilities, the home features The Hub - a central space used for activities, life skills development, and events. The Hub opens directly onto patios and landscaped gardens, encouraging indoor-outdoor living.

Further communal facilities include a therapy treatment room, an on-site hair salon, and a visitor suite, allowing friends and family to stay overnight.

Staffing levels at Hylton Grange are based on individual need and are therefore typically higher than those found in mainstream care settings.

Each unit is nurse-led, enabling an integrated approach to supporting people with high-acuity needs, including individuals who may have experienced placement breakdowns previously.

Prior to admission, a community-fit assessment is completed to ensure the home is right for the individual and that the individual is a positive fit for the existing community.

Hylton Grange includes dedicated units for adults with:
- dementia diagnoses, whose needs extend beyond the scope of mainstream providers
- neurological conditions, where mental health and behavioural needs are prominent, alongside co-occurring physical health needs
- mental health diagnoses, including individuals who may display behaviours of concern that have made other placements difficult.

Care plans are co-produced with each person and focus on personal goals, meaningful relationships, community integration, and safety.

What can I say about Hylton Grange other than excellent. The staff make a care home and the staff here, from the management team to the cleaning staff treat the residents with respect, dignity and a caring nature. They are all friendly and supportive to relatives. Aesthetically it's the nicest home
I've been in. Lovely surroundings, big rooms, nicely decorated. I'm very choosy when it comes to my husband's care but I never regretted choosing Hylton Grange. Thank you to the manager and the team.

The staff are genuinely friendly and make me feel at ease.
I don't have any concerns about leaving my relative in their care.

Since my daughter has been staying at Hylton Grange she has been given lots of support and encouragement. The staff are all very friendly and approachable which I find very helpful.
