The nurse-led team delivers high-acuity integrated care, particularly for people with co-morbidities, or those who have experienced previous failed placements.

Care plans are co-produced and focused on what matters to each person - whether rebuilding daily routines, learning life skills, maintaining hobbies, or staying connected with family.

Compared to previous care home Woodside Place is 100% better. Clean and comfortable, friendly, approachable,helpful staff. The home itself is modern, bright, clean and airy. Its the staff who make this home a great place to live for the residents going above and beyond and giving the residents a good
quality of life.

Hi, I’m Carly and I’m the Home Manager at Woodside Place in Telford.


Exemplar Health Care has a reputation for delivering first-class, person-centred care, which is achieved by truly focusing on those using the service. I wanted to get back to focusing on the individual and making a positive difference in their lives, this is something that Exemplar Health Care is known for.


I am thrilled to be able to develop what will no doubt be a strong and successful team at Woodside Place. However, I am most excited about sharing successes (big and small) with everyone who is a part of Woodside Place - we have an exciting journey ahead of us!
