Coalport Bridge Unit supports adults living with as a primary dementia diagnosis, including early onset dementia and alcohol related dementia, or Korsakoff’s syndrome, presenting with predominant complex mental health needs, and displays significant behaviours of concern, with high risk to self and others.
Iron Bridge Unit supports adults living with complex physical health needs that require a high level of nursing input, observation, and monitoring. They may have a primary physical disability or neurological
diagnosis, such as brain injury, presenting with predominant physical health needs.
The nurse-led team delivers high-acuity integrated care, particularly for people with co-morbidities, or those who have experienced previous failed placements.
Care plans are co-produced and focused on what matters to each person - whether rebuilding daily routines, learning life skills, maintaining hobbies, or staying connected with family.
Designed from the ground up to support clinical needs in a community environment, Woodside Place brings enhanced complex care capacity to Telford, helping people live meaningful lives whilst remaining close to loved ones.
