A nurse-led, multidisciplinary team delivers care, including general and mental health nurses, therapy professionals, and support staff. They work closely with each person to create bespoke care plans focused on building skills, managing behaviours of concern, and promoting autonomy.
People are encouraged to be involved in goal-setting, whether that’s improving physical function, developing daily living routines, or pursuing personal interests. Leger Grove supports hospital discharge and previously disrupted placements, helping people to establish a stable and meaningful daily life in community settings.
