Admission Criteria

  • Day Centre Admission Policy 1. Eligibility Criteria We provide services to individuals who: • Are living with dementia or memory-related conditions • Require daytime support in a structured, safe environment • May be isolated and needed Respite, help with personal care activities (mobility) • Are aged 55+ (unless early-onset dementia is diagnosed) • Do not pose a severe risk to themselves or others in a group setting We accept: • Individuals using direct payments, personal budgets, or private funding • Referrals from local authorities, GPs, or NHS memory services 2. Referral and Application Process a. Initial Inquiry Referrals can be made by: • Family members or carers • Social workers or case managers • Health professionals (e.g., dementia nurses, GPs) • Self-referral (with support where needed) b. Pre-Admission Assessment Our care team will conduct a comprehensive assessment, which includes: • Cognitive function and behaviour • Personal care needs and risk assessments • Medical history and medications • Communication abilities • Interests, routines, and life history • Mobility, dietary, and continence needs • Capacity and safeguarding concerns Assessments may take place at home or in person at our centre. 3. Admission Decision All information will be reviewed to determine: • Whether we can meet the individual’s needs safely and effectively • Any reasonable adjustments needed • Suitability of group dynamics If admitted: • A person-centred care plan will be created • A risk management plan will be developed (if needed) • The individual may begin with a trial period (e.g., 2–4 weeks) If declined: • The decision will be communicated clearly • Alternative support suggestions or referrals will be offered 4. Personal Care Delivery We provide regulated personal care in line with CQC (Care Quality Commission) standards: • All staff are trained in dementia care, safeguarding, and dignified personal care • Personal care is delivered respectfully, preserving dignity, privacy, and independence • Staff will work closely with family members and professionals to maintain continuity of care 5. Complaints & Appeals If an admission is declined or concerns arise: • Individuals or carers can raise concerns via our Complaints Policy • Appeals can be made to senior management for review
