CHAPS - Care Home Assistant Practitioner - NIGHT'S

Job Purpose

To provide high-quality, person-centred care and support to residents in a nursing home, ensuring their physical, emotional, and social needs are met with dignity and compassion. The CHAP role bridges the gap between healthcare and personal care, supporting residents’ overall wellbeing in collaboration with nursing and clinical staff.

The CHAP will actively support the delivery of a safe, effective, caring, responsive, and well-led service, assisting with care planning, documentation, and the promotion of independence while supporting the home’s leadership and compliance with policies and procedures.

CHAPS may act in a senior leader in charge during shifts, taking accountability for safe and effective care delivery and supporting junior staff in achieving high standards.

Main Responsibilities

   Clinical & Care Responsibilities

• To undertake a wide range of duties in support of the Registered Nurse in each area of the home that will ensure the highest possible standard of care is received by all residents at all times.
• To only undertake delegated duties that are within, assessed competencies and the framework of the role of the Care Home Practitioner
• To undertake clinical tasks such as physical observations, simple wound dressings, skin inspections, blood glucose monitoring, gastrostomy care, catheter care, support with the setting up and monitoring of a syringe driver
• To recognize and act on changes in residents’ conditions including changes in physical and emotional health observations using the NEWS 2 score and RESTORE 2
• Oversee nutrition and hydration management using the MUST score and other screening tools to identify malnutrition risks.
• To have a clear understanding of Dysphagia and the IDDSI descriptors
• Undertake continence management to include catheter care and stoma care and to obtain samples for analysis when required.
• Undertake management of medicines including ordering and receiving, safe storage, safe disposal, administration and auditing as requested by the Registered nurse or Home Manager
• Assist the Nurse with the planning and the delivery of good, quality end of life care. This includes managing pain, mouth care, continence and pressure area care and ensuring that the resident and their family and staff caring for the resident experience a good death.
• Ensure the wellbeing of residents is always the highest priority
• Assess, plan, implement, and evaluate individualised care plans and risk assessments to meet residents’ physical, emotional, psychological, and social needs
• Identify residents at risk of deterioration and take prompt, appropriate action
• Ensure accurate, timely, and person-centred documentation within the care planning system
• Complete regular checks on residents and provide clear, effective handovers at shift change
• Review accidents and incidents and ensure documentation, learning, and follow-up actions are completed
• Support residents with dignity, compassion, and respect, recognising their individual rights and preferences
• Encourage independence and promote choice wherever possible

·  Support residents in all aspects of their care needs, including physical, emotional, and spiritual wellbeing and  daily living activities including washing, dressing, toileting, mobility, eating, and drinking

• Assist residents with mobility aids, walking, and post-fall support
• Work under supervision and direction of the Registered Nurse
