Job Summary

The Team Leader must be competent and experienced to monitor all aspects of the day-to-day running of the Care Home to ensure that it meets its stated purpose, aims and objectives. 

Monitoring and observing is the main feature of the Team Leader role, and, Reporting to the Care Home Management, the Team Leader is responsible for supporting the Management to ensure a high level of care is provided to Residents, and support the Management to ensure that the Home meets all statutory and regulatory requirements and is aligned to codes of conduct, company Policies & Procedures and adheres to the Social Care Act.

The Team Leader will lead all the staff team, providing strong knowledge, guidance, and support, to ensure the highest standards are achieved in all areas of the home including person centred care, clinical care and dementia care.

The Team Leaders approach should promote a caring environment which provides Service Users with a high standard of Person- Centred Care, meeting individual needs and ensuring everyone is treated with respect, dignity and rights to privacy, independence and choice are met. The Team Leader will work with the Care Home Management closely, and, will also be responsible for the support and guidance of all staff and further development of the service through demonstrating strong positive ideas about Best Practice, lessons learned and Practice Improvements.

The Team Leader will be expected to support the Manager or Deputy Manager whilst the Home Management is on annual leave to ensure that there is resilience within the Management Team, and will be allocated super nummary shifts accordingly on these occasions, and will have a natural desire to learn and progress their Management skills.

Key Responsibilities · Responsibility for care in the home and smooth running of all departments. · Will support the Management with the recruitment, induction of the staff, ensuring training targets are met and the team is competent in their roles, escalating any issues to the home manager. · Will lead care delivery in the home, to ensure continuous assessment, planning, implementation and evaluation of residents’ care. · Will support Management to ensure that care plans are regularly reviewed and audited and that they are appropriate to the Resident’s needs and reflect the care being delivered. · Will support with pre-admission assessments and resident reviews, ensuring that all relevant information is transposed to the system in a person centered, and detailed manner. · Will support Management to ensure that medication is managed in line with company policies and procedures. · Will support in all aspects of induction for new starters are completed and the team is fully competent and compliant in their role and overall. · Will ensure risk assessments are completed within the home and safe working practices are being followed. Person Specification Essential and Desirable skills. · Previously worked in Care preferably at Senior Care level. · Ability to work under pressure, use initiative, improvise and effectively delegate responsibility. · Commitment to promoting and developing the highest quality care standards and having the curiosity required to look at innovative ways to improve the Homes standards. · Confidence to professionally and positively challenge poor standards / practice. · Strong, proactive communicator both verbal and written. · Can be an ambassador for the organisation, building the Homes reputation within the local community. · Drive quality assurance and compliance, lessons learned and practice improvements. · Demonstrates continuous improvement, commitment and development. · Positive role model for all staff in feedback culture and making positive changes. · Willing to be able to provide an out of hours ‘on call’ service. · Strong organizational skills. · Strong Communication skills. · Strong monitoring and reporting skills.

· Will ensure equipment, environment and practices are safe within the home, escalating any concerns as soon as they are identified. · Will support to create a ‘high performance’ positive empowering work culture with an achievement focus. To help each staff member to reflect on their practice – individually and as a team – and to assist everyone to maximize their personal growth through training, coaching and feedback. · Will assist Management with collating information for auditing processes within the Home, action plans and lessons learned to ensure continuous improvement of the Home. · Will support Management to ensure that all documentation is completed accurately and in a timely manner, reporting any shortfalls to the Management team for responsive resolve. · To spot check daily entries to ensure that documentation is clear, concise, robust and reflects the care being given. · To support with MDT meetings as required and to ensure that information is transposed to Care Plans. · Will work towards an understanding of action plans, analysis, regulatory responsibilities etc. · To assist with enquiries, admissions, pre- admission assessments as required. · To support Management to ensure that governance is maintained via auditing systems, and that appropriate communications reached each department as needed. · To support Management to prepare results and feedback from Home inspections, audit reports, Infection control visits and CQC inspections.
