Job Summary
JOB PURPOSE: To take full responsibility for providing the highest standards of Nursing Care for our Clients and empower them to live their best lives. This includes monitoring and managing the performance of, Senior Carers and Health Care Assistants and ensuring compliance with relevant professional, regulatory and contractual requirements. To ensure effective person-centred care is always delivered, maintaining the highest standards of care & support to Client’s who live in the home, whilst maintaining independence & choice in a homely environment. Promote excellent customer service and always treat people with dignity and respect. Actively ensuring the highest standard of care by managing and participating in the development of care & other allocated staff, as required. Taking full accountability for the home and the team in the absence of the Home Manager/Deputy Manager.
Main Duties and Responsibilities
Clinical Responsibilities
In this role, you’ll provide the highest standards of nursing care for our clients by:
· Providing professional, skilled and effective person centred, evidence-based hands-on nursing care
· Undertake all aspects of Nursing Care, including advanced clinical skills where the appropriate training and supervision has been obtained
- demonstrating that your practice is underpinned by clinical theory and based on a mix of therapeutic tools and intervention strategies
- acting as the named Nurse for a number of Clients
- developing collaborative needs-based care plans, through assessment, planning, implementation and evaluation
· Take accountability for administration of medication making sure that all medication including controlled drugs are ordered, stored, administered and disposed of strictly in line with The Company’s policies and procedures adhering to the NMC Code of Professional Practice and the NMC Guidelines for the Administration of Medications.
- having a sound understanding of the Oak Tree Care Group Quality Assurance framework
· Participate fully in the overall care of Client’s who live there, enabling fulfilment and a sense of belonging.
· To evaluate and review all Care plans in line with the homes policy to ensure they fully meet the required standard.
· Ensure Clients are supported to make their own decisions in accordance with the Mental Capacity Act. This will include ongoing assessment of capacity, gain Client's consent and support positive risk taking.
· Ensure the assessed needs of Client’s living in the home are regularly assessed to ensure that they receive the highest standard of care and service appropriate to their identified needs.
· Ensure each Client living in the home has an up-to-date Care plan which details the interventions needed to secure positive outcomes.

· To respond professionally and effectively to clinical emergencies in the home seeking urgent/emergency support from GP/Out of Hours services111/999 as needed.
· To promptly escalate any serious clinical issues/concerns the Home to the Clinical Lead/Manager/Deputy Manager as appropriate.
· Always act professionally, demonstrating and working within the NMC’s ‘The Code’, the Health and Social Care Act 2008 (Regulated Activities), CQC single assessment framework, Mental Capacity Act (MCA) 2005 including Deprivation of Liberty Safeguards (DOLs).
· Actively manage end of life care, ensuring people’s wishes and beliefs are respected at all times.
· Keep accurate records according to the Company’s procedures, e.g. drug administration, accidents and incidents, and care planning.
· Regularly, professionally and sensitively liaise with people’s family and friends whilst maintaining confidentiality.
· Liaise effectively with doctors, social workers, local hospitals and other community workers, to ensure the needs of our clients are met in a timely fashion.
- managing situations when people may display, or are at risk of displaying, behaviours which challenge, and knowing when to ask for assistance/advice.
· Assist the home manager in conducting robust clinical audits to ensure compliance with relevant professional, regulatory and contractual requirements, to take corrective action if required
- To assist with the delivery of food and drink to the clients.
- Ensure all complaints are dealt with appropriately in line with the company policies and procedures.
- To respond in a timely way to distressed residents (calling out, calling for help, knocking, or making noises that suggest the need for support) and to work alongside your team
Managing a shift
· Leading the Care team to deliver high standards of nursing care according to Oak Tree Care Group’s policies, practices and clinical guidelines
- planning and prioritising your own workload and the workload of the wider Care Team, delegating appropriately to ensure the smooth running of the home
· Taking responsibility for performance monitoring of care staff, including supervisions. This includes identifying required improvement and training and dealing with sub-standard performance.
· Ensure that all Care and nursing employees maintain written and electronic records in accordance with record keeping standards
· Supporting and mentoring, new nurses, preceptorship nurses and other staff as required
· Promoting and maintaining communication across the multidisciplinary team, both internal and external to the home
· Work effectively in a team
· Providing advice and learning to clients and colleagues in the home as required
· Ensuring confidentiality in respect of the clients
Maintaining Standards of Care
· Attend all statutory and mandatory training sessions, to comply with professional/legislative requirements.
· To demonstrate on going self-development, making certain that personal practice remains evidence based. Identify own learning needs in response to clinical needs of clients in the home and request training/supervision from the Home Manager as required.
· Report any concerns raised about the home by people who live there, relatives, staff or visitors to The Home Manager.
· Being familiar with and keeping up to date with the Fundamental Standards as set out in the Health and Social Care Act 2008 and Care Quality Commission (CQC) expectations for a safe, effective, caring, responsive and well-led service.
· Ensure that your own practice complies with regulatory requirements for quality and safety
· Working as an integral part of the team to ensure that the home meets CQC requirements, as well as complying with Oak Tree Care Group’s governance framework.
Health and Safety
- Ensure Maintenance/Clinical lead/Deputy/ Manager is informed of any incidents
- Ensure storage of medications is in line with company policies and procedures.
- Ensure risk assessments are completed within the home and safe working practices are being followed.
- Ensure equipment and environment are safe within the home.
